State Posting Notice Compliance Requirements:

• Must be posted in one or more conspicuous places at each insured location.
• Only employers with employees who work with tips need to post the second notice (D-22).
• Complete the designated lines at the bottom of the form prior to printing.
• Document page set up for legal 8.5 x 14” paper and may be enlarged on a copier.
• Size requirement is 11 x 17” paper.
