• Must be posted in one or more conspicuous places at each insured location.
• Notice is required to be posted in Spanish if there are Spanish speaking employees.
• Both English version and Spanish version are contained in the above attachment.
• Completing the English version will pre-fill the Spanish version.
• Select the workers’ compensation insurance carrier from the drop down box and fill in the policy period and Employer name prior to printing.
