State Posting Notice Compliance Requirements:

• Form 07-6120 (05/2012) must be completed and posted in 3 conspicuous places in every work place of the employer, including the office of the employer and the mess house or boarding house, if applicable.
• Select the Workers’ Compensation Insurance Company from the drop down box, fill in the policy period and complete the section for the Named Insured prior to printing.
