Employer’s First Report of Injury:
If you report the claim via the Internet or by telephone, you do not need to fill out this form. If you report the claim via fax, complete the form below, and fax it to Crum & Forster at the fax number provided above.

Employer’s Wage Statement:
File this form with the carrier, claimant, and claimant’s representative when the claimant is losing time from work. Read the instructions for more specifics on when to file the form.

What is the Waiting Period for Lost Wages in Virginia?
The waiting period is 7 days. If there are more than 7 days out of work, the employee is entitled to benefits from the first day out of work with a retroactive period of three weeks.

What is the Choice of Physician Rule in Virginia?
The carrier will provide a panel of three providers not associated in the same practice. Referrals beyond the initial panel choice are at the direction of the carrier unless the referral doctor has specifically named a particular doctor.
